REGISTRATION

MCCA seeks applicants that demonstrate character, curiosity, and creativity. Our application process is designed to be as simple as possible. Too often a lot of online application and registration processes can be cumbersome and frustrating to parents.   MCCA simply requires the basic information below and works on a basic mail system for registration. 

1. Fill out the form below and hit send.                                                                                                          2. We will mail you a registration form to complete with a pre-paid return envelope.                                    3. Fill out the form(s), one per child. (Vaccination Forms will be included, see vaccine post HERE)            4. Put in the supplied envelope and mail back to MCCA.                                                                              5. MCCA will contact you with questions or details on your registration.
